Notice of Dissolution of Duck Soup Coup, NFP and Instructions for Submitting Notice of Claim
You are hereby notified that Duck Soup Coup, an Illinois not-for-profit corporation (NFP), has been dissolved effective June 23, 2015. If you have an unredeemed gift certificate or other legitimate claim against Duck Soup Coup, NFP, please follow the steps below to submit your claim.
To file notice of claim against Duck Soup Coup, NFP, you must send the following information to the address below, no later than October 21, 2015.
1. A statement of the amount of your claim;
2. The legal name of the entity asserting the claim;
3. Identification and description of the agreement or other circumstances under which the claim arose; and
4. Evidence of the nature and amount of the claim, including but not limited to, promissory notes, invoices, account ledgers, statements, and contracts.

PLEASE NOTE: Failure to file notice of your claim by October 21, 2015 will result in your claim being legally barred. If your claim is so barred, you will have no further rights to assert or otherwise take action with respect to your claim, against Duck Soup Coup, NFP or any of its officers, directors, members, agents or employees.
