The Barangay San Lorenzo Business Association (BSLBA), in partnership with Philippine Statistics Authority (PSA) and MACEA, invites everyone to register for the National ID (PhilSys).

📍 Venue: MACEA Conference Room, MACEA Bldg., Legazpi cor V.A. Rufino Street, Legazpi Village, Makati City

Employees of all companies
Residents of Barangay San Lorenzo

Please bring the following:

1. Completed Application Form

(To be provided by the PSA on-site) see attached document below
2. One (1) Valid Supporting Document, such as:

Birth Certificate
Philippine Passport (e-Passport)
UMID Card
LTO Driver's License
Other equivalent documents accepted by the PSA
